Can you use medits to purchase anything from the black market or just certain items how does that work

It depends on the what the seller is willing to accept but yes, you can. Don accepts medits for a few sex products.

I have 5000 medits... how many $ is that worth? and only some products accept medits or do all?

By default, only sex products accept medits at a 500M : $1 ratio. Any other type of product, you have to ask the seller.
